Once you’ve booked your initial consultation with an OHL Physiotherapist, our admin team will send you a registration form. We kindly ask that you take a few minutes to complete it with your current medical history, relevant symptoms or concerns, weekly exercise routine, and personal goals. This helps us make your consultation more efficient and ensures we can provide care that’s tailored to your needs. You’ll also have the opportunity to upload any relevant imaging, referral letters, or medical documentation, which allows your Physiotherapist to prepare thoroughly before you arrive.
On the day of your appointment, please aim to arrive ten minutes early to allow time for check-in. We recommend wearing comfortable clothing that allows you to move freely, as this will help us assess your movement and function effectively. OHL Physiotherapists are not trained to do internal examinations, thus if you would like this style of assessment, please seek care elsewhere.
Our Physio team welcome private clients, as well as those supported by TAC, WorkCover, the Department of Veterans’ Affairs, self-managed and plan-managed NDIS, and Medicare plans with a GP referral. If you haven’t uploaded your documents in advance, please bring along any referral letters, imaging reports, or notes from your GP, obstetrician, midwife, or other health professionals involved in your care.
