Lisfranc injuries involve damage to the midfoot region, specifically the tarsometatarsal joints where the long bones of the foot (metatarsals) connect to the small bones near the ankle (tarsals). The Lisfranc ligament, which stabilises this joint complex, is often torn or disrupted during injury. These injuries are commonly seen in sports or trauma involving twisting forces or direct trauma to a plantarflexed foot—such as when an athlete falls with their foot pointed downward or someone lands on it during contact sports.
Injury severity can range from ligament sprains to fractures and joint dislocations. Symptoms include pain in the midfoot, swelling, bruising (especially on the sole), and difficulty bearing weight. Due to the subtle nature of some Lisfranc injuries, a thorough assessment by your OHL physiotherapist is essential. Clinical tests such as midfoot squeeze, single-leg heel raise, and imaging (X-ray, CT, or MRI) are used to confirm diagnosis and rule out differential conditions like metatarsal fractures, navicular injuries, plantar fasciitis, stress fractures, tendon dysfunction, and nerve entrapment.
Management depends on the extent of the injury. Stable injuries without displacement may be treated conservatively with immobilisation in a boot or cast (a plan from your trusty Sports Physician or Foot and Ankle Orthopaedic Surgeon), followed by a structured physiotherapy program. Rehabilitation focuses on restoring range of motion, strengthening intrinsic foot muscles, improving proprioception, and gradually reintroducing weight-bearing and sport-specific activities. Unstable injuries or those involving fractures often require surgical fixation, with post-operative rehab tailored to protect healing structures while restoring function.
